Purpose of going to Japan is on package tour/exhibition.
REQUIREMENTS:
1 B TOURIST
- Visa Application Form *Must be printed in A4 Size Bond Paper pdf-icon
- Philippine Passport *Broken lamination of the Photo part is not accepted. *Must have signature and have at least 2 blank pages.
- Original Birth Certificate *Must be from PSA main office/Serbilis Center issued within 1 year Photo 2×2 with white background and must be pasted on the application form
- Original Marriage Contract (if married) *Must be from PSA Main office/Serbilis Outlet Center issued within 1 year *In case of non-record you must submit certificate of non-record together with the one from the LOCAL CIVIL REGISTRAR EXEMPTION OF BIRTH CERTIFICATE AND MARRIAGE CONTRACT IF APPLICANT HAS AN OLD JAPAN VISA
- Original Copy of Bank Certificate (issued within 3 months)
- Daily Schedule in Japan (TAIZAI NITTEHIYO)
